A police report into the tabloid hacking of Milly Dowler's phone has concluded senior officers' failure to investigate the matter was "unacceptable". Officers' failure to pursue the News of the World over information gained illegally from the murdered schoolgirl's voicemail remains "a matter of deep regret" for Surrey Police, an internal investigation said.
